Case #102: The man with the foot ulcer


It has been a long time since I last produced an article containing a radiographic images. Here is one to add to the other 101 cases:

A 60-year-old man with diabetes mellitus presented with a non-healing ulcer on his left foot. Below are two images that belonged to him:

2. What is the abnormality detected in the imaging scans?

A not so clever clever patient


Not too long ago, I had a patient who has been under follow-up in my clinic for a while now who one day complained of prolonged coughing. When asked how long has the cough been, he answered, “A few months”.

There was no fever and the cough was dry. Auscultation of the chest was normal.

When asked why he did not seek advise earlier, he said, “Oh, I knew that Methadone (he used to be on Methadone to help kick his intravenous drug use habit) will suppress the cough and so for the last 2 months, I restarted myself on Methadone!”

On a hunch, we ordered an X-ray of his chest and this was what we saw:

Sigh…

The patient was immediately started on anti-TB medications. To think that he has been sitting together with many of my immuno-suppressed patients in the air-conditioned waiting room outside the clinic and coughing his lungs out makes me shudder.

Case #94: Suddenly breathless


This 24 year old woman felt an uneasy discomfort on the left side of her chest from morning which worsened progressively. By noon, she decided to go to the hospital. When she presented to the hospital, she was in distress, having suddenly developed acute breathlessness. She collapsed and was resuscitated requiring mechanical ventilation.

This Xray was taken a short while before she collapsed.

Case #93: The man with shortness of breath


This is the chest radiograph of a young man with HIV. He complains of progressively worsening shortness of breath of 3 weeks duration with occasional low grade fever. On examination, he is tachypnoeic, afebrile, and auscultation of the lungs reveals occasional scattered crepitations. His oxygen saturation on room air is 72%.
