1. Oblique fracture of proximal part of 2nd, 3rd and 4th metacarpal, and fracture-dislocation of 2nd distal interphalangeal joint.
2. K-wiring done and a Volar slab in place.
Kudos to: AC, CK, frhyh, and am.
LOL-award to mac and lishun!
1. oblique fracture of proximal part of 2nd, 3rd and 4th metacarpal, and fracture-dislocation of 2nd distal interphalangeal joint.
2. fixed with K-wires and immobilized witha back-slab.
Left hand radiograph AP view: (This radiograph is taken at the anatomical position with the thumb at the left of the radiograph & marker placed at the right side)
1) 2nd DIP fracture dislocation.
2) Oblique fracture of the 2nd, 3rd 4th Proximal metacarpals.
3) Severe hand swelling – presumable open fracture as air pockets could be seen.
4) Adult hand
5) Bone quality normal
Procedure done: K-wire insertion with back slab. His left wrist is placed on an abducted position suggestive for possible suspected scaphoid fracture too.
1. Right hand crush injury. Transverse # of the 2,3,4 metacarpal. ?fracture dislocation of the 2nd PIPJ.
Possible cause of trauma
- wife hitting with a golf club
- the hand stuck in air tebu machinery
- Ah Long related violence
- or the usual motor vehicle accident
1. Displaced transverse fractures of the 2nd, 3rd and 4th metacarpals shafts. Also something weird is going on with the PIP joint of the index finger. Dislocated maybe.
2. K-wires + backslab plaster
1. Ah long broke borrower’s hand with an iron hammer / mallet / pipe – soft tissue injury + 2,3,4 metacarpal #. Accurate enough to miss the carpals and phalanxes.
2. Ah long implants multiple tracking devices in form of criss-crossing K-wires or generic “dawai”, and not decided to coat the hand with cement.

1. displaced fracture of 2nd/3rd/4th shaft of metacarpal of right hand. oblique #.. mech of injury: maybe direct blow..
2. 2 K-wires placed across the # and put on backslab in angulated position
psstt… dr.J.. portfolios r killing me softly… =(
